White Plains Center for Nursing Care (NY) — CNA Neglect and Cover-Up, Dementia Resident Elopement (2017)
Outcome: Two certified nurse aides at White Plains Center for Nursing Care — Janet Rose, 62, and Verlanda Paulma, 25 — were arrested December 20, 2017 on felony endangering charges after failing to conduct required monitoring checks on a resident with dementia who eloped from the facility, and allegedly covering up their monitoring failures.
On May 26, 2017, an at-risk resident with dementia wandered away from White Plains Center for Nursing Care in White Plains, New York. The two certified nurse aides — Rose and Paulma — allegedly failed to perform required monitoring checks on the at-risk resident, allowing the elopement to occur undetected. The case escalated beyond simple negligence to potential criminal conduct when investigators found evidence of an alleged cover-up of the monitoring failures.
New York Attorney General Eric Schneiderman announced the arrests on December 20, 2017. Both defendants faced felony charges of Endangering the Welfare of an Incompetent or Physically Disabled Person in the First Degree and related charges. Nursing homes are required to maintain individualized elopement prevention plans for residents identified as at-risk, including scheduled monitoring checks with documented completion.

How Crucible Prevents This
Crucible's elopement monitoring controls and incident documentation hooks ensure that monitoring check failures are recorded and reported before they become criminal concealment. Automated supervision reminders and time-stamped monitoring logs create accountability for high-risk residents. Cover-up attempts are harder to sustain when digital records capture the absence of required monitoring.
