Kino Insurance & Tax Services, Inc. operated as a dual-purpose insurance and tax preparation business in South Gate, California (Los Angeles County), serving the local community. Between 2011 and 2018, owner Maria Aquino accepted insurance premium payments from clients and then failed to remit those premiums to insurance carriers — a classic premium diversion scheme. The clients believed they were purchasing active insurance coverage, but no policies were actually placed on their behalf.

To maintain the appearance of legitimate insurance coverage and delay client discovery of the fraud, Aquino falsified certificates of insurance for more than eight clients — creating fraudulent documents that stated active policies were in force when no such coverage existed. This meant clients who presented their certificates of insurance to landlords, lenders, or other parties requiring proof of coverage were presenting fraudulent documents without knowing it.

The California Department of Insurance revoked Aquino's insurance producer license on July 12, 2018. The Los Angeles County District Attorney's Office prosecuted the case following the CDI investigation.
