Heather M. Entrekin, D.V.M. — DEA Registration Revocation for Lack of State Authority (2022–2023)
Outcome: DEA issued an Order to Show Cause to Heather M. Entrekin, D.V.M. on August 9, 2022, and revoked her DEA Certificate of Registration No. FE4914164 effective April 21, 2023, on the ground that she was without authority to handle controlled substances in Alabama, the state in which she was DEA-registered.
On August 9, 2022, the Drug Enforcement Administration issued an Order to Show Cause to Heather M. Entrekin, D.V.M., because she lacked authority to handle controlled substances in the State of Alabama, the state in which her DEA registration was held. An individual must hold an Alabama controlled substance license to dispense a controlled substance in Alabama, and because Entrekin lacked that state authority, she was not eligible to maintain a DEA registration.
Entrekin timely requested a hearing. The Government filed a Motion for Summary Disposition, and the Chief Administrative Law Judge granted the motion, recommending revocation of her registration. The DEA adopted the recommendation. The DEA revoked DEA Certificate of Registration No. FE4914164 issued to Heather M. Entrekin, D.V.M., and denied any pending applications to renew or modify the registration, as well as any other pending applications for additional DEA registration in Alabama, with the order effective April 21, 2023.
The final Decision and Order was published in the Federal Register on March 22, 2023.

How Crucible Prevents This
The DEA revoked this veterinarian's registration because she lacked state-level controlled substance authority — a status that should have been continuously monitored. Crucible's license and registration status monitoring would have flagged the lapse in Alabama controlled substance licensure and triggered an immediate review before the DEA acted.
