Three affiliated skilled nursing facilities in Illinois — Symphony of Chicago West, Symphony of Morgan Park, and Symphony Midway — agreed to pay $300,000 to resolve allegations they violated the False Claims Act by billing Medicare for medically unnecessary rehabilitation therapy services between 2014 and 2019.

The United States alleged that between January 2014 and September 2019, the facilities billed Medicare for physical therapy, occupational therapy, and speech pathology services that were provided longer than medically necessary and without regard to individual patient needs. The facilities used inflated Resource Utilization Group (RUG) levels to maximize Medicare reimbursement, putting profits over appropriate patient care.
